Neurotology Subspecialty Examination Application Requirements Exam Day Information Exam Guidelines Hotel Information   Exam Fee Exam Fee: $4,640 Exam Day Information The Neurotology Exam will be administered on Monday, April 18, 2011 at the Westin O'Hare. The registration location will be announced at a later date. A government-issued photo ID (driver's license, passport, state ID card) is required for registration. You are NOT permitted to bring personal items (briefcases, backpacks, purses, books, PDAs, copying devices, cell phones, beepers, pens, and pencils) in the testing area. Luggage must be checked at the bell stand in the hotel lobby. Failure to report on time may disqualify you from the examination. Please dress professionally (jacket and tie for men; business attire for women). To avoid any bias we ask that you not wear your military uniform. Please have breakfast or lunch prior to the exam. Only beverages will be available at registration. Hotel Information A block of rooms has been reserved for candidates at a special rate of $165 (plus tax) per night, single or double. This rate represents a discount over standard rates. Please reserve your room early as they fill up quickly. Guests can access the site to book, modify, or cancel a reservation until March 23, 2011.
